Unable to add users, groups, or other entities from web admin

Hi,

I followed the online docs for installing kolab-3.0 on a fresh debian 
wheezy install.  I was able to add my first user and make that user a 
kolab-admin.  As I was poking around the roundcube interface, I tried to 
change the password for my user and this seemed to fail (after I click 
save I get a blank page and I have to use my old password to login to 
roundcube).  Also, now I cannot edit this users information or add new 
users/groups/other entities from the kolab-webadmin interface, even when 
logged in as Directory Manager.  I cannot seem to find anything 
complaining in the logs except this:

What have I broken and where can I look for how to fix it?
