Add external calendar

Hi,

we are working on merging the upstream code for Kolab 3.3. Until then, 
we only support roundcubemail-plugins-kolab-3.1.13 from Kolab 3.2:

If you checkout the HEAD of the feature-caldav branch, you'll get the 
latest progress for the Kolab 3.3 merge. But be warned: This is highly 
unstable code which is not event tested yet.
