for me, this works:

kdepim-3.5.6.enterprise.0.20070814.700060$ diff -Nur \ 
       kaddressbook/main.cpp.orig kaddressbook/main.cpp \
       >debian/patches/31_kaddressbook_main_cpp.diff

kdepim-3.5.6.enterprise.0.20070814.700060$ debian/rules apply-patches
[...]
Trying patch debian/patches/27_dimap_warning.diff at level 0...1...success.
Trying patch debian/patches/30_cvs-sh-version.diff at level 0...1...success.
Trying patch debian/patches/31_kaddressbook_main_cpp.diff at level
