folders not showing up in Roundcube and as "notes" in Outlook

Here is some more weirdness.

I am running Kolab Enterprise on CentOS 6.4 with the Bynari Connector.

There is a folder hierarchy used to organize saved emails.

This folder listing shows up fine in Thunderbird.

In Roundcube, I can see the folder listing under Settings, Folders and 
the folders are properly subscribed to.

In Roundcube, in the mail area, the folders are not visible.

In Outlook, the folders are visible, but messages are shown as "Notes".

Any ideas?
