First, I am pretty new to Kolab so I'll try to describe things as best
as I can :) I hope I'm not too verbose.

I'm testing this off a fresh install of Kolab on CentOS 6.4 using the
kolab meta package and the setup-kolab script. The
/etc/roundcubemail/main.inc.php has kolab_folders listed in both the
$rcmail_config['plugins'] array, and the $mandatory_plugins array.
There are no "isolated domains" setup or files being pulled by the
include statement in the middle of the file.

The kolab_folders.inc.php file exists and Inbox, Drafts, Sent, Spam,
and Trash defined along with default Calendar, Contact, and
Configuration folders. Again, no additional files are being included
with the include statement.

Inside Roundcube I can see that in the Settings => Special Folders, all
of the boxes looked appropriately defined.

I setup a Motorola XyBoard (Android 4.0.4) and it does the same thing.
Only the Inbox is synced by default (along with Calendar, Contacts,
Tasks). It's also having another odd issue but I'll test that a few
times and post a more specific message for it :)
