imapsync very poor performance

Dovecot index should reside on a local storage.
If using nfs, try this settings in the fstab:

rw,noatime,nordirplus,nolock,fsc,timeo=60,actimeo=5,rsize=65535,tcp,wsize=65535,hard
Check, that you have no stale NFS handles, by using ls or find in the
users home.
