I have seen this due to a crashed openldap.
I was running openpkg on debian and I got ldap error messages
in /var/log/messages.
There was also a crashed instance of kolabd.
After stopping openpkg, I used ps -A and kill to remove kolabd, slapd and
multiple kolabquotawarns.
Restarting the openpkg system resumed normal operation.
