There must be a difference between a working system and yours.
Best would be if you had a similiar clean system right beside it so 
you can most easily compare.

Next I would probably compare the LDAP schemas to a healthy system.
Then turn on kolabd logging 
next try the debugging mode or openldap
then debug the php code in more detail.
