What do you mean by "extensions" here?
Offline capabilities are a great feature of the Kolab Concept.
People would want it for projects and their documents as well.
So a concept should consider this for the future.

Smart Clients are also central in the Kolab philosphy, 
because they shift processing to the client
and make the server more scalable.

Each IMAP email client can already save documents as attachments,
so technically this would be the first place to add a different interface 
to access them.

My email was a brain-dump on the potential design issues.
It is a second question of what to do next.

An experiment to see if a webdav readonly access to email-attachments is 
suited to see where this design path leads to.
It could be done using Horde as Gunnar pointed out or something simpler
using high level languages and their rich libraries (e.g. Python email 
and imap modules).
This would be a prototype and not solve all use cases.
