You did add horde.schema into slapd.conf, right? And select LDAP as your 
preference backend within Horde?

This looks like a missconfigured LDAP server, or incorrect preference system 
parameters specified within Horde. In the Preferences tab within the Horde 
configuration screen, make sure that the "Username search key" is set to 
"mail" (without the quotes). Also, the Base DN, Root DN and Root DN password 
within this screen must be identical to the ones you specified in the Kolab 
tab.

On an unrelated note, something quite curious is that fact that all your 
emails to this list have their "Date" header set to Wed, 30 Jun 2004 10:46:57 
-0400.
