It seems that only the "Automatically send delivery notifications" check box has
any effect on MDNs.  All settings in the "Message Disposition Notifications"
group are ignored.  More specifically:

If "Automatically send delivery confirmations" is on, a Receipt is sent if
requested by the sender by checking "Options/Request Disposition Notification"
in the KMail composer.  It is always sent, regardless of the settings in the
"Message Disposition Notifications" group.  The receipt always includes the
headers of the original mail.

If Automatically send delivery confirmations" is on, receipts are never sent,
regardless of the settings in the "Message Disposition Notifications" group.
