Kontact settings for ldap

I have my kolab server working (more or less out of the Mageia 3 box) and both Roundcube and 
Thunderbird is doing well, even sending on port 387.
But I still have problems with Kontakt/Kmail
I cannot get the LDAP Server Settings.

Can someone create a screen shot of a working setup, including the username and password
What password is required, the e-mail user or another one
Security TLS?
Authentication simple?
