Does Kolab meet my (non-exclusive) requirements?

I don't think that Outlook does z-push and that's not a USB Connection either.

Outlook speaks MAPI and of course you can use one of the connectors without 
conflicts. Everything syncs against the server (that's whay a local USB 
connection won't work).

Yes. Kontact for Windows ist still beta, but quite usable, thanks to Andre's 
hard work.

I have not tested that one yet.

Did you try setting it to the correct folder type? Once the type is correct, 
the contacts should be displayed fine.
