I'm not really certain if I can give you useful advice on installing
Altermime since I never used this. But I'd expect that you need to
specify the content filter for the smtpd running on port 465:

465       inet  n       -       n       -       -       smtpd -o smtpd_tls_wrappermode=yes -o smtpd_sasl_auth_enable=yes -o content_filter=dfilt:

I'm not entirely certain that this will work though ;)
