Can you have a look at 

/var/log/messages (ie. the system log)
/kolab/var/openldap/openldap.log
/kolab/var/sasl/log/saslauthd.log
/kolab/var/imapd/log/master.log
/kolab/var/imapd/log/misc.log

and see if there is anything fishy in there?

The webinterface and the creation of the IMAP mailbox are completely 
separate. The webinterface only creates an account object in the LDAP 
database. This triggers the mailbox creation in another process shortly 
after.
