    Add own setting dialog for kolab resource
    
    * add own defaults for kolabresource
      -> m_settings, onConfigurationDone  must be protected to be used from
      koloab resource
    
    * modify setting dialog for kolab
      - add kolab icon for dialog
      - do not show "offline use" checkbox
